Office Policies
Co-Pays, Co-Insurance, Deductibles and all balances are due and payable upon check-in.
A $15.00 fee will be charged if you do not pay your co-pay in full at your visit. This is necessary due to the added administrative expenses in having to bill you.
A $50.00 cancellation fee will be charged to any patient cancelling an appointment in our office with less than 24 hours-notice.
A $150.00 cancellation fee will be charged to any patient cancelling a procedure in the Ambulatory Surgery Center with less than 72 hours-notice.

Please note: It is our suggestion that you verify with your insurance carrier whether or not you any deductible, co-insurance or co-pay related to the use of the office or the ambulatory surgery center. We recommend that you call your insurance company to verify insurance participation status. For procedures, the endoscopist, facility, anesthesiologist and pathologist are all separate charges that will be billed for service rendered. If we do not participate with your insurance plan, your out-of-network benefits [if applicable] will be utilized and you will be responsible for any deductibles, co-insurance or co-pays as dictated by your insurance plan.
