*Empire:
Please be advised that this plan has a $200.00 out of pocket deductible for
in network outpatient facilities or outpatient hospital department charges.
You are responsible for the amount indicated by Empire on the explanation
of benefits form that we receive.
**Oxford:
If you have Oxford and have purchased an individual policy, you may have a
deductible for in network outpatient facility / hospital charges. You will
be billed for the amount indicated by Oxford on the explanation of benefits
form that we receive.
